How it's stored
Submissions are delivered by email within our Google Workspace account, which is covered under a signed Business Associate Agreement (BAA) with Google. No third-party form service has access to your information.
Cookies & tracking
This website does not use cookies or tracking to collect health information. If a minimal analytics or advertising cookie (such as Google Ads conversion tracking) is enabled, it is used only to measure general site traffic and form submissions — never to track diagnoses, treatment, or other health details. A cookie banner lets you accept or decline before any such cookie is set.
