The contact form on this website collects your name, email address, phone number, state of residence, and any message you choose to write. Please do not submit protected health information (PHI) — such as diagnoses, medications, or treatment details — through this form.
How it's used
Information submitted through the contact form is used only to respond to your inquiry. It is not sold, shared with advertisers, or used for marketing outreach.
How it's stored
Submissions are delivered by email within our Google Workspace account, which is covered under a signed Business Associate Agreement (BAA) with Google. No third-party form service has access to your information.
Cookies & tracking
If you accept the cookie notice, this site loads Google Tag Manager, used only to measure whether the contact form was submitted (a conversion count) for our advertising campaigns. We do not enable remarketing, ad personalization, or audience-building features, and no health information is ever shared with Google.
Your rights
You may request that we delete any information you've submitted by emailing us using the contact information below.
